The Outcomes Report calculates how practitioners have performed based on patient progression using standardised questionnaires.
What can you use this Report for?
Generate this report to display the progression of clients who have completed the same questionnaire multiple times.
Filters and Options
| Filters | Description |
| Date | The report displays all data for which at least one questionnaire from the sequence was filled within the date range |
| Providers | The provider who sent the questionnaire for the first time |
| Minimum no. of datapoints | The minimum number of results for the same questionnaire to display. |
| Outcomes | Select which questionnaire to include in the report |
What is displayed in the report?
- The summary table displays each provider in a separate row with the number of clients that have filled questionnaires, total completed outcome questionnaires, percentage improvement per visit and percentage overall improvement.
- The details table list each that has completed outcomes questionnaire within the date range.

| Provider | The provider who sent the questionnaire the first time for the course of treatment. |
| Client | Client Name who completed the questionnaire |
| Metric | Name of the questionnaire completed |
| Case | Case Title for which the questionnaire was completed |
| Gender | Client's gender |
| DOB | Client's date of birth |
| First Report Date | The date the questionnaire was completed for the first time |
| Last Report Date | The date the most recent questionnaire was completed |
| Improvement per Visit | Percentage of improvement per measure is calculated by total improvement from initial to most recent results divided by the number of time the questionnaire was completed |
| Overall Improvement | Percentage of improvement from the initial result to the most recent result |
